Case Manager - 90 day opportunity
Job Description:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing CPS investigations support and guidance to individuals in need in DeKalb County, helping them navigate through various challenges and connect with necessary resources. Duties:
- Conduct comprehensive assessments to identify clients' needs and develop individualized care plans
- Collaborate with clients to set goals and monitor progress towards achieving them
- Provide counseling and support services to individuals dealing with addiction, mental health issues, or other behavioral health concerns
- Assist clients in accessing housing resources, including Section 8 vouchers and affordable housing options
- Coordinate with community organizations and agencies to ensure clients receive necessary services, such as workforce development programs or crisis management support
- Advocate for clients' rights and needs within the healthcare system, legal system, or other relevant institutions
- Maintain accurate and up-to-date documentation of client interactions, progress notes, and service referrals
Requirements:
- Bachelor's degree in social work or related field; master's degree preferred
- Proven experience in case management or related roles
- Knowledge of social work principles, behavioral health interventions, and motivational interviewing techniques
- Familiarity with housing assistance programs, such as Section 8
- Strong organizational skills and ability to manage multiple cases simultaneously
- Excellent communication skills, both verbal and written
- Ability to work collaboratively with diverse populations and community partners
