Case Manager - 60 day opportunity
Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families in navigating complex systems, providing guidance, resources, and advocacy to ensure their needs are met. This position requires a strong understanding of social services, crisis intervention, and behavioral health to effectively assist clients in achieving their goals.
Duties
- Conduct assessments to determine client needs and develop individualized service plans.
- Provide crisis intervention and support to clients facing immediate challenges.
- Facilitate access to resources such as childcare, housing assistance (including Section 8), and healthcare services (including Medicare).
- Collaborate with community organizations and service providers to coordinate care and support for clients.
- Monitor client progress and adjust service plans as necessary to ensure effective outcomes.
- Offer addiction counseling and behavioral health support as needed.
- Maintain accurate documentation of client interactions, progress notes, and case files.
- Supervise interns or junior staff members when applicable, providing guidance and support in their roles.
Skills
- Strong knowledge of social work principles and practices.
- Proficiency in crisis management techniques and strategies.
- Experience with child welfare systems and regulations.
- Familiarity with addiction counseling methodologies.
- Excellent communication skills, both verbal and written.
- Ability to work collaboratively with diverse populations and multidisciplinary teams.
- Strong organizational skills with attention to detail in case documentation.
