Job Overview

We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families in navigating complex systems, providing guidance, resources, and advocacy to ensure their needs are met. This position requires a strong understanding of social services, crisis intervention, and behavioral health to effectively assist clients in achieving their goals.

Duties

  • Conduct assessments to determine client needs and develop individualized service plans.
  • Provide crisis intervention and support to clients facing immediate challenges.
  • Facilitate access to resources such as childcare, housing assistance (including Section 8), and healthcare services (including Medicare).
  • Collaborate with community organizations and service providers to coordinate care and support for clients.
  • Monitor client progress and adjust service plans as necessary to ensure effective outcomes.
  • Offer addiction counseling and behavioral health support as needed.
  • Maintain accurate documentation of client interactions, progress notes, and case files.
  • Supervise interns or junior staff members when applicable, providing guidance and support in their roles.

Skills

  • Strong knowledge of social work principles and practices.
  • Proficiency in crisis management techniques and strategies.
  • Experience with child welfare systems and regulations.
  • Familiarity with addiction counseling methodologies.
  • Excellent communication skills, both verbal and written.
  • Ability to work collaboratively with diverse populations and multidisciplinary teams.
  • Strong organizational skills with attention to detail in case documentation.

Pay: $33.50 per hour
