Job Summary

We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy. The Case Manager will work collaboratively with clients to develop personalized plans that address their needs and goals, ensuring access to necessary services and support systems.

Responsibilities

  • Conduct comprehensive assessments of clients’ needs, strengths, and challenges.
  • Develop and implement individualized service plans in collaboration with clients.
  • Utilize motivational interviewing techniques to empower clients in their decision-making processes.
  • Provide ongoing case management support, including monitoring progress and adjusting plans as necessary.
  • Facilitate access to community resources such as Medicare, Section 8 housing, childcare services, and workforce development programs.
  • Offer crisis intervention services to clients experiencing immediate challenges or emergencies.
  • Mentor clients to enhance their skills and promote self-sufficiency.
  • Supervise and support interns or volunteers involved in case management activities.
  • Maintain accurate and confidential client records in compliance with organizational policies and regulations.
  • Collaborate with multidisciplinary teams to ensure comprehensive care for clients.

Qualifications

  • Bachelor’s degree in Social Work or a related field.
  • Proven experience in case management or social services is highly desirable.
  • Familiarity with child welfare systems and practices is a plus.
  • Strong skills in crisis intervention, mentoring, and supervising others.
  • Excellent communication skills, both verbal and written, with the ability to engage effectively with diverse populations.
  • Knowledge of community resources related to healthcare, housing, employment, and social services is essential.
  • Ability to work independently as well as part of a team-oriented environment.
  • Proficient in using technology for documentation and communication purposes.

Job Type: Full-time

Pay: $33.50 per hour

Expected hours: 40 per week
