Location : 445 Winn Way Decatur GA

Hours: Part-time ( 25 hours weekly)

Position Summary: 

Under limited supervision of the Chronic Disease Prevention Coordinator, the Community Health Worker will use evidence-based strategies, best practices, and programs to address the growing burden of chronic diseases and associated risk factors, such as poor nutrition, lack of physical activity, and tobacco use. This position will provide coordination of chronic disease prevention activities utilizing a policy, systems, and environmental change approach and a health equity lens. This position participates in the administration of program grants, contracts, and budgets. Monitors program activities for compliance with state and federal program guidelines. 

Job Description and Duties: 

• Provide client management (blood pressure monitoring, control support, education, etc.) for persons diagnosed with primary hypertension. Diagnosis based on current Joint National Commission clinical guidelines for detection, management, and treatment of hypertension. This service may be collaboratively offered with private providers.

• Support community outreach events: recruitment of patients, conduct screening and provide health education and promotion activities.

• Serves as liaison for the HPI, Live Healthy Del<alb Coalition, and the community.

• Participates in outreach events such as health fairs and community events to promote the increased rates of vaccinations.

• Develops and maintains relationships with internal and external partners.

• Works with community organizations, coalitions, schools, and healthcare systems to address policy and environmental changes.

• Collaborate with partners to determine community health needs and the availability of services and to develop goals for meeting needs.

• Attends required training and other pertinent meetings, organizes lectures, and hosts educational conferences and seminars.

• Maintains knowledge of current trends and developments in the field by reading appropriate books, journals, and other literature, attending related seminars and conferences, and applying pertinent new knowledge to the performance of other responsibilities.

• Maintains databases, mailing lists, telephone networks, and other information to facilitate the functioning of health education programs.

• Other duties as assigned.
