Job Description:
The Dietitian will be assigned to care for approximately 20-30 patients (hereinafter referred to as " clients” or " consumers”).
The Dietitian will be scheduled to work approximately 20 hours per week during daytime hours Monday through Friday. It
is preferred for the Dietitian to work four hours per day during morning hours, five days per week, but HCPH may allow
other schedules (such as eight hours per day on two days and four hours per day on one day per week).
The Dietitian;s duties will include but not necessarily be limited to the following.
• Completing initial and annual nutrition assessments of all assigned consumers
• Following up with consumers monthly or quarterly depending on nutrition risk
• Providing individualized interventions based on consumers' nutrition/health goals
• Attending and participating in interdisciplinary care of consumers
• Communicating with Food Service staff about special dietary needs of assigned consumers
• Facilitating weekly nutrition-based groups with consumers
• Approving weekly menu changes
The Dietitian must be currently licensed as a Dietitian in Missouri and must maintain registration with the Commission on
Dietetic Registration.
The Dietitian will be required to attend HCPH orientation and training. Orientation will be conducted during daytime hours,
between approximately 8:00 a.m. and 4:30 p.m., Monday through Friday.

Remarks:
The Dietitian will be required to attend orientation and training. The contractor shall be paid at the rates quoted in the
Pricing section of this RFQ for the time the Dietitian spends in such orientation and training. Orientation will be conducted
during daytime hours, between approximately 8:00 a.m. and 4:30 p.m., Monday through Friday.
