Physical Therapist (1099) 

Pay rate: $51.50 per hour (1099)

Job Description:

  • This is a licensed professional position in Physical Therapy responsible for treating injured, ill, or disabled individuals
  • through therapeutic use of everyday activities to help individuals develop, recover, improve and maintain the skills needed for daily living.

Current needs for the position referenced are as follows:

  • Physical Therapist – averages 40 hours/month

The Contractors personnel shall adhere to the following requirements:

  • Utilizes Electronic Healthcare Record (EHR) system.
  • Follows Department Operation Regulations (DORs), the client policies and procedures, and ICF guidelines.
  • Acts with independence in directing screening and assessment, treatment, intervention, administration, program planning, and documentation for acute and long-term care patients to enable them to improve their optimal functional abilities.
  • Applies clinical reasoning in developing physical therapy plans and programs.
  • Knowledge of principles and methods used to develop and implement individual treatment plans and the use of adaptive equipment designed to assist in self-help tasks.
  • Ability to understand, interpret, and adapt a physician's written or oral directions utilizing evidence-based treatment options; and to plan, assign, and supervise the work of subordinate therapists and aides.
  • Ability to plan, organize, lead, and adapt planned therapeutic activities in accordance with patient's needs.
  • Communicates timely and regularly in writing with individuals, the Interdisciplinary Team (IDT), co-workers, and/or supervisors utilizing agency forms, computer programs and email as required.
  • Implements and carries out individual treatment/therapy plans and assists in developing and teaching living skills related to self-help, recreation, socialization, meals, etc. and escorts individuals as assigned.
  • Responds to health care needs for individuals by observing, reporting changes in physical condition, behavior and emotional response.

  • Must be knowledgeable of the current principles, procedures, techniques, and instrumentation used in evaluating or treating injured, ill, or disabled patients through therapeutic use of everyday activities.
  • Must direct screening and assessment, treatment, intervention, administration, program planning, and documentation.
